You can fax or mail your orders to be paid via Visa, Mastercard, Discover, or PayPal. If paying by check or money order simply mail to the address below. You may print out this order form by clicking the button below. Your billing address is that shown on your credit card/debit account statement. Upon receipt of your order, we will send you a confirmation email invoice for your records. If we will be shipping to a California address there is a 9.25% sales tax on the total product value, not including shipping fees. There is no sales tax for shipments to other states. The shipping charge should be calculated as 20% (or one-fifth) of the total value of all products in your order if those products total more then $100 in value. The shipping charge should be calculated as 25% (or one-fourth) of the total value of all products in your order if those products total $60 to $100 in value. The shipping charge should be calculated as 30% of the total value of all products in your order if those products total under $60. There is a $10 minimum shipping fee.
